Shoulder pain can make simple activities such as wearing clothes, combing your hair, reaching for an object or sleeping comfortably difficult. Two common conditions that may cause these problems are frozen shoulder and rotator cuff injury.
Although both conditions can cause pain and restricted movement, they affect different parts of the shoulder and require different treatment approaches. A frozen shoulder mainly causes progressive stiffness in the shoulder joint, while a rotator cuff injury often causes pain and weakness, particularly when lifting or rotating the arm.
Understanding the differences can help you recognise when it is time to seek medical evaluation.
Understanding the Shoulder Joint
The shoulder is a ball-and-socket joint that allows the arm to move in multiple directions. A flexible layer of connective tissue called the shoulder capsule surrounds the joint.
The rotator cuff is a group of muscles and tendons that stabilises the shoulder and helps lift and rotate the arm. Frozen shoulder affects the joint capsule, while a rotator cuff injury affects one or more of these muscles or tendons.
What Is a Frozen Shoulder?
Frozen shoulder, medically known as adhesive capsulitis, occurs when the capsule surrounding the shoulder joint becomes thick, tight and inflamed. Bands of stiff tissue may develop, making the shoulder increasingly difficult to move.
The condition usually develops gradually. Pain may appear first, followed by increasing stiffness and loss of movement. Recovery is often slow and may take several months or, in some cases, up to three years.
What Is a Rotator Cuff Injury?
A rotator cuff injury occurs when one or more of the tendons surrounding the shoulder become irritated, inflamed, strained or torn.
The injury may develop gradually because of ageing or repeated overhead movements. It may also occur suddenly after a fall, lifting a heavy object or making a forceful movement with the arm.
Rotator cuff problems can range from mild tendon irritation to a partial or complete tendon tear.
Frozen Shoulder vs Rotator Cuff Injury: The Main Difference
The most important difference is how each condition affects shoulder movement.
With frozen shoulder, both active movement and passive movement are restricted. Active movement means moving your arm by yourself, while passive movement means another person moves your relaxed arm.
With many rotator cuff injuries, actively lifting the arm may be painful or weak, but another person may still be able to move the shoulder through a greater range. However, severe or long-standing rotator cuff problems can also lead to stiffness, so a proper examination is necessary.

Symptoms of Frozen Shoulder
Frozen shoulder commonly causes:
- A dull or aching pain in the shoulder

- Difficulty raising the arm
- Difficulty rotating the arm outward
- Trouble reaching behind the back
- Difficulty wearing clothes or fastening garments
- Difficulty combing or washing the hair
- Reduced movement even when someone else tries to move the arm
Pain is often felt over the outer shoulder and may extend into the upper arm. The defining feature is that shoulder movement becomes limited in almost every direction.
The Three Stages of Frozen Shoulder
Frozen shoulder usually progresses through three stages.
1. Freezing Stage
During this stage, shoulder pain gradually increases and movement becomes more restricted. The freezing stage may last from approximately six weeks to nine months.
2. Frozen Stage
The pain may begin to reduce, but the stiffness remains. Everyday activities can become particularly difficult during this stage.
3. Thawing Stage
Shoulder movement slowly begins to improve. Recovery of movement and strength can take several months or longer.
Symptoms of a Rotator Cuff Injury
A rotator cuff injury may cause:
- Pain in the front or side of the shoulder
- Pain that travels down the upper arm
- Pain while lifting or lowering the arm
- Difficulty reaching overhead
- Weakness when lifting or rotating the arm
- Pain while lying on the affected shoulder
- Night-time shoulder pain
- A crackling or clicking sensation
- Difficulty combing the hair or reaching behind the back
A sudden rotator cuff tear may cause intense pain, a snapping sensation and immediate weakness after a fall or forceful movement. Tears that develop gradually may initially cause mild pain during overhead activities before becoming more noticeable at rest.
Does Weakness Mean a Rotator Cuff Injury?
Noticeable weakness is more strongly associated with a rotator cuff injury, particularly when you try to raise the arm away from the body or rotate it.
However, people with frozen shoulder may also feel that the arm is weak because pain and stiffness prevent normal use. This is why weakness alone cannot confirm the diagnosis.
A doctor may perform specific strength and movement tests to determine whether the limitation is caused by joint stiffness, tendon damage, pain or another shoulder condition.
What Causes Frozen Shoulder?
The exact cause of the frozen shoulder is not always known. It may develop without a clear injury, but certain factors increase the risk.
These include:
- Diabetes
- Thyroid disorders
- Prolonged shoulder immobilisation
- Recovery after shoulder or arm surgery
- A previous fracture or injury
- Parkinson’s disease
- Certain cardiac conditions
Frozen shoulder most commonly affects adults between 40 and 60 years of age and is more frequently reported in women. People with diabetes may experience greater stiffness and a longer recovery period.
What Causes a Rotator Cuff Injury?
Rotator cuff injuries generally occur because of either sudden trauma or gradual wear.
Possible causes and risk factors include:
- Falling onto an outstretched arm
- Lifting a heavy object with a sudden movement
- Repetitive overhead activity
- Age-related tendon degeneration
- Playing sports involving repeated arm movements

A rotator cuff problem may also become difficult to identify when severe stiffness is present. Since the conditions can overlap and are sometimes mistaken for one another, a detailed shoulder examination is important before beginning treatment.
How Are These Conditions Diagnosed?
The doctor will ask when the symptoms began, whether there was an injury, which movements cause pain and whether the shoulder feels weak or stiff.
During the physical examination, the doctor may:
- Check active shoulder movement
- Check passive shoulder movement
- Test the strength of the arm
- Look for areas of tenderness
- Observe how the shoulder moves
- Examine the neck to rule out nerve-related pain
Frozen shoulder can often be identified through the pattern of restricted active and passive movement. Imaging may be used to rule out arthritis, fractures or rotator cuff damage.
Are X-rays, Ultrasound or MRI Required?
Not every patient needs every imaging test.
X-ray
An X-ray can help detect fractures, arthritis and other bone-related causes of shoulder pain. It does not clearly show the rotator cuff tendons.
Ultrasound
An ultrasound can show the soft tissues of the shoulder and may identify a rotator cuff tear.
MRI Scan
An MRI provides detailed images of the tendons, muscles and other soft tissues. It can help determine the location, size and severity of a rotator cuff tear.
Frozen shoulder does not always require an MRI for diagnosis. However, an MRI or ultrasound may be recommended when the doctor suspects an associated rotator cuff injury or another soft-tissue condition.
Treatment for Frozen Shoulder
Treatment focuses on controlling pain and gradually restoring shoulder movement.
Pain Management
Pain-relieving or anti-inflammatory medicines may be recommended based on the patient’s health and symptoms.
Physiotherapy
Physiotherapy is a central part of frozen shoulder treatment. Gentle stretching and range-of-motion exercises can help improve flexibility and prevent further loss of movement.
Exercises should be performed according to medical or physiotherapy guidance. Forcing the shoulder through severe pain may aggravate symptoms.
Steroid Injection
A corticosteroid injection into the shoulder joint may reduce pain and improve movement, particularly during the earlier stages.
Hydrodilatation
During hydrodilatation, sterile fluid is injected into the shoulder joint to stretch the tightened capsule and improve movement.
Surgery
Surgery is rarely required for frozen shoulder. Manipulation under anaesthesia or arthroscopic surgery may be considered when severe symptoms continue despite appropriate nonsurgical treatment.
Treatment for a Rotator Cuff Injury
Treatment depends on the type and size of the injury, the patient’s age, activity level, general health and the severity of weakness.
Nonsurgical treatment may include:
- Temporarily resting the shoulder
- Avoiding painful overhead activities
- Applying ice as advised
- Pain-relieving or anti-inflammatory medication
- Physiotherapy
- Strengthening and flexibility exercises
- An injection in selected cases
Many patients experience reduced pain and improved shoulder function with nonsurgical care. Surgery may be considered for complete tears, significant weakness, acute traumatic injuries or symptoms that do not improve with conservative treatment.

Frozen shoulder treatment focuses heavily on restoring joint movement, while rotator cuff rehabilitation may place greater emphasis on protecting and strengthening injured tendons.
Beginning unsuitable exercises without knowing the cause may worsen pain or delay recovery. A correct diagnosis helps the doctor or physiotherapist select exercises that are appropriate for the condition and its stage.
